Description

HOURS: Monday through Friday 8:00 a.m. – 5:00 p.m.

AREA SERVED: Bexar County and Surrounding Counties (45 miles from office)

SERVICES OFFERED: MCH Family Outreach in San Antonio offers foster care and family preservation
services through its community services program. Services are designed to help meet the
needs of families with one or more children between the ages of 0 to 17.

PROGRAM DESCRIPTION: MCH Family Outreach strengthens families to help them provide a safe, stable
and healthy family environment.

The Family Solutions program helps families by providing advocacy and support, connection to
resources in the community and other services. Services are designed for families with at least
one child residing in the home between the ages of birth to 14 years.

Foster care provides a temporary living arrangement – typically ranging from 6 to 18 months – until
the parents are able to resume parenting. Children in care range from birth through 17 years of
age. Very few babies or children have special needs, such as medication or counseling.

The GAP Program (Grandparents as Parents) provides services to grandparents and other relatives
who care for children in the absence of parents. We support families by providing in-home
assessments, case management services, information and referrals, support groups, assistance
with legal issues and parenting workshops

Parent Education: Our parent education programs follow an instructional and supportive framework
aimed at increasing nurturing skills and reducing the potential for abusive or neglectful
behaviors.

ELIGIBILITY REQUIREMENTS: Eligible families should also live within a 45-mile radius of an MCH
Outreach office.

APPLICATION PROCEDURE: Call 210-733-3904

COST TO CLIENT: All services are provided at no cost.
